How to set up an evaluation
The first step in requesting a therapy evaluation is to obtain a referral from your child’s Primary Care Physician (PCP). Prescriptions for therapy may be faxed from your doctor’s office or dropped off at our clinic. We will then contact you to set up an evaluation time.
If you would like to fill out paperwork beforehand, you can print off the New Patient Forms. Otherwise, plan to arrive fifteen minutes prior to your appointment.
We encourage parents to call their insurance provider to obtain benefits before the evaluation. Please bring a copy of your insurance card and/or Arkansas Medicaid card, if applicable.
